This role partners closely with clinical team members to empower patients to better understand and manage their heart failure through prevention-focused, patient-centered RESPONSIBILITIES:<br><br></span></strong></span></span></span><span><span><span><strong><span>Patient Education & Engagement<br></span></strong></span></span></span> <ul><li><span><span><span><span>Provide education and coaching related to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management.</span></span></span></span></li><li><span><span><span><span>Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques.</span></span></span></span></li><li><span><span><span><span>Encourage sustained patient engagement, self-advocacy, and behavior change throughout program required.</span></span></span></span></li><li><span><span><span><span>Experience in a healthcare, chronic disease management, or patient engagement setting preferred.</span></span></span></span></li><li><span><span><span><span>Cardiology or heart failure experience a plus.</span></span></span></span></li><li><span><span><span><span>Telehealth or remote
