Nurse Practitioner/PA (Remote 7PM - 7AM, Fri/Sat/Sun)

We are seeking a dynamic and compassionate Nurse Practitioner or Physician Assistant to join our multidisciplinary healthcare team in this fully remote position. In this pivotal role, you will provide high-quality, patient-centered care for multiple subacute rehab and skilled nursing facilities. Your expertise will support comprehensive assessments, diagnosis, treatment planning, and ongoing management of patients.

This position is for the night shift 7PM - 7AM (3 shifts x 12 hours - Friday, Saturday and Sunday).

This position requires a current/valid New York NP/PA license and preferably already a Pennsylvania and Massachusetts license as well.

Responsibilities:

  • Cover our on-call provider line, which currently covers multiple subacute rehab and skilled nursing facilities in across multiple states.
  • Review labs, radiology reports, medications, consults, pharmacy recommendations.
  • Prescribe medications including narcotics and controlled substances when needed.
  • Prescribe and order a variety of IV medications, IV fluids.
  • Order diagnostic testing when needed (labs, x-rays, fluids, UA etc.).
  • Send to ED or refer to specialists when needed.
  • Other remote work as delegated by Director of Remote Provider Services

Requirements

  • Valid New York state licensure as a Nurse Practitioner or Physician Assistant with current certification (e.g., NCCPA certification for PAs).
  • Current/Valid New York DEA license
  • Ideally a Pennsylvania and Massachusetts Nurse Practitioner or Physician Assistant as well
  • Proven experience in subacute rehab and skilled nursing facilities
  • Strong familiarity with EMR/EHR systems

Job Type: Full-time

Pay: $125,000.00 per year
